You really don't need a form because services that are investigational or experimental are deemed to be excluded benefits under the patient's benefit plan. In other words, they are non-covered...

Are you sure you're reading your contract correctly? I've never seen a contract written to prohibit a provider for charging the patient for non-covered services. Not to say it's not happened, of course!
